The Positive Behavior Support Facilitator will work with the consumer and their team to build positive behaviors which is outlined in the person centered Positive Behavior Support Plan. The behavior plan will include ways to reduce the challenging behaviors. The consumer is instrumental in this process as the focus is placed on what is important to and for the consumer. Once, the Facilitator has written the plan, it will then be shared with the consumer and their team. The Facilitator will review and train the team as how to implement the plan. Should re-training be needed the Facilitator is able to provide this.
